During First Collaborative Committee Meeting


First Collaborative Committee Meeting may be the first of the year or the first of its kind for that student.

  • Tier 2: Skill Building Form
  • Members in Attendance
  • Concern to Address
  • Instructional & State Accommodations*
  • Frequency of Progress Reporting
  • Comments → relevant data that lead to determination of RtI support (i.e. STAR 360, BAS, i-Station, behavior, etc)
  • Upload documentation to student’s History tab*
  • Print copy of form with Parent letter; distribute letter to parents
  • Collaborative Committee members

* These items may or may not be relevant to the student’s RtI/MTSS needs (i.e. behavior vs. academic data collection)


** If the Collaborative Committee recommends for a student to be moved to Tier 3, then a new Tier 3: Intensive Intervention Form will be created. See the Students Entering to Tier 3 document for more information on which data to include. The completed Tier 2 form will be locked.


**If the Collaborative Committee recommends for a student to remain in Tier 2, then the committee will use the current form to add new strategy to intervention plan
